Ketostix detect the presence of acetone in the urine. Acetone is a volatile compound with a quite low boiling point, so it is readily exhaled from the body as gas and thru the urinary tract. It is feasible to be in a low level of ketosis in which acetoacetate mainly synthesizes beta-hydroxybutyrate, and does not fall to forming acetone. It is also possible to use up all of the blood-ketones thru extreme exercising so that there is no excess acetone to escape into the urinary tract. Another possibility is that the urine is so dilute from excess water consumption that acetone doesn’t register on the strip even if it is there if your urine is clear and you aren’t registering for ketosis, then this is a quite great possibility.
